  1. I agree to review the Champaign Unit 4 Schools COVID-19 Operational/Safety Protocols document with my student prior to their return to in-person learning.  I will also review any updates, as needed.

  2. I will encourage my student to follow all of the required safety protocols, including wearing a mask as required.

  3. I agree to complete the Unit 4 Symptom Screen Form each day and send it with my student to school. (see page 3 in Operational/Safety Protocol manual for an example).

A copy will be mailed home with Report Cards for students to bring on the first day of in-person learning.  On the first day of in-person learning, students will be provided with copies to take home.

  1. I agree to arrange transportation, within one (1) hour, for my student if they develop any symptoms associated with COVID-19. I understand that if I am unable to arrange transportation, my student may return to distance learning.

By sending your student to in-person learning, you are agreeing to these expectations. If you are unable to follow the protocols and pick-up students on time, we will ask students to return to distance learning to support everyone’s safety.

Barkstall staff are ready to welcome your children.  Yes, school will look different, but we look forward to seeing our students again.  Please if you have any questions, feel free to contact the school office at 217-373-7330, or email me directly. I am happy to chat and answer any questions.
